Mariela heads an organization called the Cuban National Center for Sex Education (CENESEX). The government-funded organization has been educating the public on contraception, AIDS prevention, and educational campaigns about sex and gender equality. With Mariela at the helm, CENESEX successfully pushed through a bill to have sexual reassignment surgery covered by government health plans in 2008.
